About the Program: The Transitional Support Program (TSP) is a Supported Apartment Program for young adults aged 18-25. Staffed 24/7 by a multidisciplinary team—including an Employment Specialist, Therapist, Residential Counselors, Administrative Assistant, Residential Supervisor and Program Director—TSP provides services focused on teaching daily living, social and recreational skills, time management, interpersonal skills, conflict resolution, coping skills, and symptom management. These services are delivered through both group and individual modalities to help young adults transition to independent living.

Duties & Responsibilities:
- Maintain physical environment and program standards in compliance with CHR, DMHAS, JCAHO, and other licensing standards
- Monitor medications of all residents according to the Adult Residential Medication Policy and Procedure
- Effectively communicate and transfer information to other staff regarding daily events
- Teach and assist residents with nutrition, health, meal preparation, hygiene, laundry, banking, budgeting, and basic home‑care skills
- Assist and manage administrative tasks at the direction of the supervisor
- Prepare and facilitate educational and skill‑building groups and one‑to‑one interventions as directed by treatment plans
- Collaborate and coordinate care with providers and families as needed
- Assist residents in addressing medical and psychiatric needs
- Maintain documentation in compliance with agency and program standards
- Participate in staff meetings, supervision, trainings, and other meetings as assigned
- Perform additional duties as required
Qualifications:
- High school diploma or GED required
- Preferred: two years of experience working with a relevant population
- Valid driver’s license with a good driving record required
Compensation: Starting at $17.00/hr minimum, with rates determined at the time of offer based on experience and internal equity policy. Eligible for program/location differentials and sign‑on incentives. Part‑time positions are prorated based on hours worked.
