Full Time Outpatient Physical Therapist, Neuro
A Physical Therapist will evaluate, diagnose, and manage patients with physical impairments and functional limitations to maximize the patient’s functional abilities. The Physical Therapist will design and implement individualized treatment plans that address specific goals and needs of each patient, and educate patients and their families on prevention and management of conditions to promote optimal health and wellness.
Job Duties & Responsibilities- Performs comprehensive initial evaluations to identify functional deficits and develops a treatment plan.
- Provides skilled interventions tailored to patient needs and preferences, using appropriate techniques, modalities, and equipment.
- Collaborates with patients to create short and long‑term goals.
- Communicates effectively and respectfully with patients, families, and other health care providers, ensuring informed consent, confidentiality, and cultural sensitivity.
- Manages patients to improve mobility, reduce or manage pain, restore function, and prevent disability through direct or indirect treatments or referrals.
- Establishes and updates diagnosis, prognosis, and plan of care based on the best available evidence and clinical reasoning.
- Reviews and records information in the patient’s medical record, including evaluative findings, treatment plans, responses to treatment, and status changes.
- Educates patients and family members about prognosis and care options.
- Assists in developing and implementing department programs.
- Participates in quality improvement initiatives, continuing education activities, and professional development opportunities.
- Monitors and documents patient progress, outcomes, and satisfaction.
- Shares clinical knowledge and new techniques to improve patient care.
- Supervises Physical Therapy Assistants and PT/PTA students as assigned.
- Maintains confidentiality of all materials and ensures proper release of information.
- Complies with network and departmental policies on safety and reporting requirements.
- Adheres to ethical, legal, and regulatory standards of the physical therapy profession.
- Demonstrates excellence in service performance and continuous improvement in clinical care, customer service, operations, and safety.
- Displays financial responsibility and accountability in use of resources.
- Complies with attendance and dress code policies.
- Demonstrates competency in assessment, treatment, growth and development knowledge, and age‑appropriate communication.
- Attends meetings as designated.
- Accurately bills patient accounts for services rendered.
- Provides in‑services to PT department staff.
- Participates in QI data collection and planning.
- Attends multidisciplinary meetings as applicable.
- Maintains supplies and assists facility director with daily operations.
- Other related duties as assigned.
Must be able to sit for up to ninety minutes, stand for thirty to forty‑five minutes consecutively, and manipulate dials on machines using hands/fingers. Must occasionally stoop, bend, squat, crouch, kneel, and reach above shoulder level. Must lift/move patients up to 50 lb and push/pull patients weighing up to 50 lb. Must have tactile sensation for muscle spasms, texture, and skin temperature, normal hearing for conversation, and general vision with depth and peripheral perception.
EducationDoctoral Degree, Master’s, or Bachelor’s degree from an accredited physical therapy program or a physical therapy certificate.
Training and ExperienceSuccessful completion of required affiliations. A minimum of one year of experience as a physical therapist in an outpatient setting.
