Physical Therapist Assistant
Care Well Health Medical Center is seeking a dedicated Physical Therapist Assistant to evaluate and treat patients in a collaborative environment.
Job SummaryResponsible for evaluating and assessing the needs of referred patients and formulating evidence‑based treatment plans. Provides therapy services as defined in treatment plans while working cooperatively with physicians, case managers, and adjustors. May supervise physical therapy assistants and aides and has a certain degree of creativity and latitude required.
Essential Functions- Meet patients’ goals and needs by assessing and interpreting evaluations and test results, determining and implementing physical therapy treatment plans.
- Help patients accomplish treatment plans and accept therapeutic devices through manual exercises, instruction, encouragement, and assistance with functional activities, using assistive devices such as crutches, canes, and prostheses.
- Apply physical agent modalities as needed to meet patient and treatment goals.
- Evaluate effects of treatments by observing progress, recommending adjustments and modifications.
- Ensure continuation of therapeutic plans post‑discharge by designing home exercise programs and instructing patients, families, and caregivers.
- Participate in discharge planning by consulting with physicians, nurses, social workers, and other health‑care workers.
- Practice established hospital and departmental policies, quality improvement programs, safety, environmental, and infection‑control standards.
- Document evaluations, re‑evaluations, goals, and plans accurately and timely; complete required forms in accordance with company policy and regulations.
- Maintain professional knowledge through workshops, publications, and professional societies.
- Comply with federal, state, and accrediting‑body requirements.

Minimum Education / Certifications- Master’s degree in physical therapy from an accredited college or university (unless graduated before January 1 2003 from an accredited program with a bachelor’s degree or certificate).
- Doctor of Physical Therapy preferred.
- Licensed Physical Therapist by the New Jersey Department of Consumer Affairs.
- BLS certification from American Heart Association.
At least 3 months of experience in acute/subacute or outpatient physical therapy treating orthopedic, neurological, or medical populations.
Position Type / Expected HoursFull‑time, with willingness to work holidays, weekends, and provide additional coverage as needed.
Physical Requirements- Lift, transport, and transfer patients and equipment.
- Walk, stand, and bend for extended periods.
- Provide heavy manual resistance while exercising patients.
- Good manual dexterity and adequate visual and hearing acuity.
Occasional exposure to cleaning/disinfecting chemicals and possible exposure to communicable diseases.
