Job Summary
Substance Abuse Counselor is responsible for the provision of substance abuse counseling services, assigned assessments, assisting with direct admissions, participating as part of the treatment team, and instrumental in discharge planning.
Essential Functions- Screening and Diagnosis
:
Determining the appropriate level of care according to ASAM Criteria;
Diagnosing clients for substance use disorders using the DSM-5 - Assessment and Treatment Planning
:
Conduct thorough assessments of clients' substance use, mental health, and readiness for change, and to develop comprehensive, client-centered treatment plans. Must be competent in CIWA and COWS assessment tools. - Counseling Services
:
Provide individual, group, and family therapy, teach coping skills, and address behaviors interfering with recovery. - Documentation
:
Maintain accurate and timely medical records, including progress notes, treatment plans, and discharge summaries, in compliance with agency, state, and federal standards. - Collaboration
:
Work with our multidisciplinary treatment team and other community agencies, such as medical facilities, support groups, and housing programs, to ensure coordinated care. - Crisis Intervention
:
Provide crisis intervention services to support patients. - Referral and Linkage
:
Assist clients in accessing appropriate resources, including self-help programs, residential facilities, and other community services. - Aftercare and Discharge
:
Develop and implement discharge plans and aftercare strategies to ensure continued support and prevent relapses after treatment.
- Facilitate individual, group, and family therapy sessions.
- Conduct client and family education about addiction and recovery.
- Liaise with referral sources and other agencies.
- Participate in clinical staff meetings and treatment planning reviews.
- Assist in the transition to healthier lifestyles by addressing self-esteem, social adjustment, and coping skills.

2 year of substance use disorder/behavioral health work experience preferred.
