Position SummaryThe Transition Coordinator for Special Education is responsible for developing, coordinating, and expanding community-based transition opportunities for students with disabilities, with a primary focus on internships, hands‑on job experiences, and community-based learning. This role supports compliance with IDEA transition planning requirements and ensures students are provided meaningful post‑secondary preparation aligned with their Individualized Education Programs (IEPs).
The Transition Coordinator works collaboratively with district staff, families, community providers, and local businesses to create sustainable partnerships that promote career readiness, independent living skills, and successful post‑school outcomes.
