Case Manager | Skilled Nursing Facility

Overview

We are seeking a qualified Case Manager / Discharge Planner to support care coordination and discharge planning in a Skilled Nursing and Rehabilitation Facility. While the role is focused on case management, candidates with a Social Work background are strongly preferred.

Pay Range: $64,000.00 - $67,000.00 per year

• Manage case management and discharge planning from admission through discharge
• Coordinate care transitions to home, rehab, assisted living, or other settings
• Communicate with residents and families regarding discharge plans and available resources
• Collaborate with nursing, therapy, physicians, and the interdisciplinary team
• Arrange post-discharge services including home care, DME, transportation, and follow-up care
• Work with insurance providers and external agencies as needed
• Ensure accurate documentation and regulatory compliance

• Bachelor’s or Master’s degree in Social Work preferred
• LMSW preferred but not required
• Experience in case management or discharge planning in a SNF, hospital, or healthcare setting required
• Strong understanding of care transitions and resident advocacy
• Excellent communication, organization, and problem-solving skills
