We are seeking a qualified Case Manager / Discharge Planner to support care coordination and discharge planning in a Skilled Nursing and Rehabilitation Facility. While the role is focused on case management, candidates with a Social Work background are strongly preferred.
Pay Range: $64,000.00 - $67,000.00 per year
Key Responsibilities:Manage case management and discharge planning from admission through discharge
Coordinate care transitions to home, rehab, assisted living, or other settings
Communicate with residents and families regarding discharge plans and available resources
Collaborate with nursing, therapy, physicians, and the interdisciplinary team
Arrange post-discharge services including home care, DME, transportation, and follow-up care
Work with insurance providers and external agencies as needed
Ensure accurate documentation and regulatory compliance
Bachelor’s or Master’s degree in Social Work preferred
LMSW preferred but not required
Experience in case management or discharge planning in a SNF, hospital, or healthcare setting required
Strong understanding of care transitions and resident advocacy
Excellent communication, organization, and problem-solving skills
