Overview
Flagstar Nursing is seeking Physical Therapist's (PT) for Skilled Nursing Facilities! Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Long Island. Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate: $46 Hourly (W-2), $57 Hourly (1099)
Responsibilities- Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
- Supervise PTAs daily.
- Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress with the use of innovative EMR software.
