Overview

Position Summary:

Social Services Director will abide by and practice according to the Facility Policies and Procedures Manual, state and federal guidelines, and the National Association of Social Workers code of ethics. Social Service functions include, but are not limited to:
Social Services staff, direct services, advocacy, counseling service coordination, education, in-servicing, policy and program development and community relations.

Responsibilities/Accountabilities
  • Plans and directs comprehensive social services program which provides for support services for residents and families
  • Participates in an interdisciplinary team approach to better serve the needs of the nursing center population
  • Provides for therapeutic interventions to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities as well as their feelings about institutionalization and separation from family/community, physical and emotional
  • Maintains written documentation in the resident's medical record per the Facility policy, refer to the Facility Social Services Policies and Procedures Manual and State and Federal Guidelines
  • Performs an initial evaluation/assessment of each resident s biopsychosocial needs to place in the medical record. Written documentation at least quarterly is required for each resident. Active cases require ongoing written documentation. For required written documentation, the Facility Social Services Manual, State and Federal Guidelines are to be followed.
  • Develops social service component of the president s plan of care, identifying specific problems, goals and approaches. Participates and coordinates per facility policy, interdisciplinary care planning conference at time of admission and at prescribed intervals thereafter in order to help develop and revise the overall plan of care for resident. Ensure and documents the residents/families have input into the care planning process
  • Acts as a liaison between residents, families, outside agencies and the facility Administrator to ensure that the resident rights are maintained
  • Takes responsibility for conducting family group meetings as needed (new admissions group, support group, family and friends  council, etc.)
  • Assumes the role and function in admission coordinator for the absence of the Admission Director
  • Functions in a management capacity and adheres to all policy/procedures of the facility, as a representative of the Facility administration
  • Performs administrative requirements such as completing necessary forms and reports under the directions of the Administrator and submitting such reports to the Administrator and/or consultants as required
  • Facilitates discharge planning including the development of an organized discharge plan for all residents
  • Concerns himself/herself with the safety of all residents in order to minimize the potential for fire and accidents. Also, ensures that the facility adheres to legal, safety, health, fire and sanitation codes and is familiar with current MSDS
  • Puts customer service first. Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual s needs and rights
  • Perform other duties as requested
Job Skills
  • Knowledge of casework process, needs of geriatric residents, medical setting and working knowledge of funding sources such as Medicare, Medicaid and other insurances
  • Imagination and creation ability, skill in working cooperatively with other professional, staff, residents and facilities. Ability to organize and carry responsibility efficiently and effectively
  • Ability to communicate effectively and appropriately both verbally and in writing in a wide variety of circumstances
  • Commitment to philosophy and objectives of the Facility
  • Knowledge of governmental regulations relating to social service delivery
  • Provide annual verification of negative TB skin test
Perform Related Duties
  • Interact with residents, families, visitors and Facility subsidiary personnel
  • Carries out other tasks as required in situations where hands-on intervention/participation may be required
Specific Education/Vocational Requirements
  • Master s degree or bachelor s degree in social work preferred. May also have a bachelor s degree in human services field including, but not limited to, Sociology, Gerontology, Counseling and Psychology
  • Needs to possess any certified/licensures required by State regulations
