Medical Coordinator for La JornadaThe Medical Coordinator arranges and coordinates the proper delivery of medical, dental, mental health and pharmacy services on behalf of children in care, including urgent, emergent and preventive health care. The Coordinator accurately documents services, maintains confidential records, communicates with care teams and coordinates appointments and interpretation services, striving to meet continuous quality improvement measures.
Responsibilities- Perform all work in a manner consistent with NYAP’s mission, values and philosophies.
- Provide children with a positive and appropriate medical environment consistent with NYAP’s mission, values and philosophies.
- Ensure confidentiality of health records and adhere to NYAP, State and Funder PHI guidelines.
- Work to obtain MOUs with medical, dental and mental health providers.
- Participate in intakes and discharges of children, including administering COVID‑19 testing and preparing medical documentation packages.
- Continuously assess children’s medical, dental and mental health needs and coordinate care.
- Collaborate with children’s assigned cross‑functional team and providers to ensure appropriate care, including scheduling appointments and obtaining medical clearances.
- Report medical emergencies via SIR/CLE process.
- Elevate medical concerns to the supervisor, cross‑functional team and DHUC for guidance.
- Support foster parents by providing education on medical policies and answering questions.
- Collect forms and health records from providers and upload them into portals.
- Complete and submit all assigned documentation within required time frames.
- Regularly communicate with children’s care teams and meet continuous quality improvement measures.
- Maintain medicine cabinets and first aid kits with essential items.
- Provide in‑service training and ongoing education to staff and foster parents.
- Maintain Red Cross CPR/First Aid Trainer certifications.
- Assist Program Leadership during auditing processes.
- Perform other duties as requested.
- LPN/LVN or higher health‑related qualification (e.g., RN with a bachelor’s degree) OR bachelor’s degree in a relevant field.
- Bilingual fluency in English and Spanish is required.
- Minimum of 1 year of employment in public health or a healthcare setting with experience in care coordination, health administration or direct clinical care.
- 1 year of experience working in the child welfare field with children and/or adolescents in a social service setting.
- Valid driver’s license, excellent driving record, clean personal vehicle and current automobile insurance.
- Compliance with required training programs.
- Proficient use of desktop,, smartphone, tablet and related software (word processing, spreadsheets, databases).
- Experience scheduling medical appointments.
- Experience documenting medical appointments.
- Recognizes and understands the needs of the immigrant population, providing support and creating a safe environment for the children in the program.
- Demonstrated ability to maintain liaisons with community health providers and expand community health network.
- Must be open to and willing to work non‑traditional hours and be available for on‑call phone consultations as needed.
- Manual dexterity, tactile, visual and audio acuity.
- Repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands.
- Occasional lifting (up to 25 pounds), bending, pulling and carrying.
- Ability to travel frequently and drive vehicle while sitting for extended periods, with frequency varying based on program demands.
