Position Description
- Plan and provide clinical support to In-Home/Residential patients.
- Provide and coordinate patient care, educate patients.
- Conduct new client intake.
- Educate and train staff on required medical trainings.
- Document client care services by charting in client records.
- Administer and audit medications and medication administration records.
- Perform wellness checks as needed.
- Coordinate with client’s interdisciplinary team, maintaining cooperative relationships by communicating information, responding to requests, and building rapport.
- Be flexible and adjust to the needs of the program.
- Attend interdisciplinary team (IDT) meetings.
- Preferred experience working with individuals with developmental disabilities.
- Possess a valid New Jersey Registered Nurse License.
- Valid New Jersey Driver’s License.
- Ability to push, pull, and lift up to 50 lbs.
