What you will do
  • Assess and plan patient discharge in collaboration with a multi-disciplinary team
  • Develop personalised occupational therapy programmes to facilitate efficient patient discharge
  • Communicate effectively with patients, families, and healthcare providers to ensure comprehensive discharge planning
  • Monitor and evaluate patient progress and modify plans accordingly to support successful discharge
  • Maintain accurate and up-to-date patient records and reports
Requirements
  • HCPC registration is essential
  • Possession of a driving license is advantageous to accommodate community visits and maximise outreach
