What You Will Do:

Conduct holistic assessments to identify individual needs and create tailored care plans.

Recommend and arrange adaptations
, including minor and major home modifications, to support independent living.

Advise on postural seating, moving and handling
, and provide equipment to enhance daily functioning.

Offer rehousing recommendations for clients requiring more suitable accommodation.

Work collaboratively with a dynamic multidisciplinary team to deliver efficient, person-centred support.

Requirements:

HCPC registration as an Occupational Therapist.

Full UK driving licence
, with the ability to travel to community visits.
