Responsibilities

Provides diabetes education and nursing services to the Native American community as part of a comprehensive health program. Provides services as specified in the grant for Special Diabetes Program for Indians (SPDI). Completes the required data for the Diabetic Audit Report (DART) and submit report timely.

Essential Duties and responsibilities include the following:
  • Coordinates diabetes education programs/services within the guidelines of the Kickapoo Community Health Center and Indian Health Services Standards of Care.
  • Conducts home visits as directed by the Director of Nursing or Medical Provider for health care education to orient diabetic patients to self- care practices.
  • Gathers and documents information about the patient’s condition, including but not limited to: vital signs, medications, allergies, pain levels, glucose logs, monitoring equipment, physical activity, knowledge of disease, food in the home, etc. Reports any concerns or abnormalities to the medical provider.
  • Monitors glucose levels in accordance to protocols, reports results as indicated and documents orders received accurately.
  • Perform nursing skills in a competent and safe manner and in accordance to clinic policies/procedures.
  • Reviews medications as prescribed by the provider; instructs patients about their medications or treatments; administers medications and/or injections, such as insulin, as ordered by the provider in a timely and appropriate manner.
  • Reviews lab values including Hemoglobin A1C results and ensures timely testing of blood and/or urine tests in accordance to SDPI grant requirements.
  • Assists the medical provider in referring patients to specialists such as nutrition, dentistry, podiatry and ophthalmology as well as to physical activity programs when ordered.
  • Works collaboratively with all health care team members to assist the diabetic patient in attaining diabetes self-management.
  • Establishes a diabetes team and meets regularly to discuss barriers to diabetes self-management of patients and create solutions for improvement.
  • Organizes diabetes classes, individually or in groups, to patients referred by the Kickapoo Community Health Center Clinical Team or Medical Provider.
  • Participates in the development of patient educational materials for use by the Diabetic Program as approved by the Medical Director.
  • Collaborates with other team members in developing and implementing community wellness/prevention programs.
  • In-services clinical staff on Diabetes Program Requirements and other identified needs to ensure compliance with the SDPI and DART programs.
  • Provides services and support to the community through participation in health fairs, public speaking, creating support groups, etc.
  • Reviews the electronic health record to complete the Diabetic Form, as required by the DART and SDPI program; updates the electronic health record regarding any pertinent communication and/or changes in treatment. Maintains the Diabetes registry with data collection to meet the SDPI and DART program outcome measures and objectives; prepares project narrative and submits required reports in a timely manner and as required by the Indian Health Services grant.
  • Prepares and submits written reports of service activities, narrative and statistical reports as requested by administration.
  • Maintains familiarity with current national standards developed by the American Diabetes Association and/or the American Association of Diabetes Educators.
  • Participates in establishing and conducting Quality Improvement activities.
  • Maintains professional knowledge and proficiency through continuing education, in-services and workshops.
  • Maintain standard and ethics at all times, to include compliance with applicable privacy regulations, if any.
  • Employee must maintain confidentiality, security and integrity of patient data and information according to applicable privacy regulations, if any.
  • Employee assures age and culturally appropriate, holistic patient care based on mission, standards and policies; adheres to standards of relevant regulatory agencies.
  • Employee participates in clinic performance improvement activities to measure, assess and improve the quality of patient care.
