Responsibilities

In a manner consistent and supportive of our values, the Primary Care Provider is responsible for delivering high quality healthcare within Fast Pace Health's scope of services while achieving optimum patient satisfaction. The Primary Care Provider is responsible for preventative care for the patients as their first point of contact and takes continued responsibility for providing the patient's ongoing comprehensive care, focusing on their long-term health and well-being.

The Primary Care Provider must be able to work in a team-oriented environment, be flexible to adapt to new technologies and protocols in a quickly evolving practice setting. They must have the ability to respond quickly and accurately to changes in condition or response to treatment and is responsible for providing outstanding patient service within the clinic and through various virtual communication channels, while maintaining a compassionate and welcome atmosphere.

Primary Care Essential

  • Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up.
  • Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma.
  • Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings.
  • Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs.
  • Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans.
  • Prescribe medications, recommend treatments, and provide written home-going instructions.
  • Refer patients to specialists or other healthcare resources as needed.
  • Educate patients and their families on health maintenance and preventive care.
  • Promote patient engagement, health literacy, and shared decision-making.
  • Participate in quality improvement initiatives and value-based care programs.
  • Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships.
Essential Functions
  • Ability to provide quality care and meet patient volume goals (approximately +18 patients daily).
  • Lead clinic staff in delivering excellent patient care and provide guidance to ensure quality professional services and patient satisfaction.
  • Review patients' medical history, symptoms, allergies, and current medications to formulate accurate diagnoses and provide guidance.
  • Engage with clinical leadership to ensure strong patient care and adherence to company values.
  • Ensure accurate completion of patient charts in a timely manner and forward charts as appropriate.
  • Work efficiently in a fast-paced autonomous environment.
  • Dedication to evidence-based medicine and staying current with medical advancements.
  • Implement clinical protocols as outlined by CMO and Supervising Physicians.
  • Perform responsibilities within standard NP/PA protocols.
  • Analyze and interpret patient histories, symptoms, findings, and diagnostic information to develop diagnoses.
  • Deliver excellent patient care with attention to patient-centric care, safety, cost, and accuracy of information.
  • Ensure compliance with department expectations and regulations, including health information management issues (HIPAA).
  • Complete mandatory compliance training and adhere to core values of teamwork, communication, empowerment, quality of care, and friendliness.
  • Suggest improvements to enhance patient experience and care; maintain open communication within the team.
