We are looking to add a new Midlevel Provider to our quickly growing clinic (NP/PA).

- Address patients' clinical concerns (symptom reporting, medication refills, lab clarification).
- Address critical lab/imaging results with patients and re‑route medications to pharmacies (including controlled substances) as needed.
- Assist with clinical operational tasks such as referrals, lab/imaging orders, provider letters, and medical forms.
- Serve as a clinical resource and professional role model for other staff and assist with coaching of newly hired providers.
- Reduce conflict and increase patient satisfaction whenever possible through timely response to patient phone calls, chats and tasks; conduct medical chart reviews.
- Maintain accurate, clear and concise progress notes, problem lists, and medication lists.
- Provide patient education regarding disease management, therapies and health‑ful behaviors.
- Communicate appropriately and tactfully with staff, consultants, patients, significant others, and community.
- Participate in staff, planning, in‑service, and other meetings as needed.
- Perform other duties as assigned.
- Board Certified Physician Assistant or Family Nurse Practitioner.
- Active license in Tennessee and willingness to be licensed in other states as we grow.
- Active DEA with all schedules and in good standing.
- Willingness to learn how to diagnose and treat Chronic Kidney Disease.
- Typical schedule is Monday through Thursday 8 AM‑4:30 PM, Friday 8 AM‑1 PM.
- Must be able to work ONE WEEKEND DAY per month between 9 AM‑5 PM.
- Must have availability Monday‑Friday to cover shifts between 6 AM‑9 PM.
