Overview

Social Service Director – Dyer Nursing and Rehabilitation Center is looking for a high level Social Services Professional to lead our team. Social Services is that integral part of our interdisciplinary team, working with therapy, nursing and the rest of the team to ensure the quality of the residents stay and then a smooth transition back into their home life. At Dyer Nursing and Rehabilitation Center, we value the contributions of every care provider and team member.

• Develop an ongoing care plan based on the residents’ needs, interests, and abilities as identified in the initial assessment.
• Participate in care plan meetings.
• Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy.
• Implement healthcare directives & identify end of life care opportunities.
• Assess the resident's potential for discharge as a member of an interdisciplinary team.
• Plan for the resident's discharge and post-discharge with the resident, family, and interdisciplinary team.
• Assist the resident and family in the implementation of the discharge plan.

• Bachelor's degree in Social Work
• 2-3 years’ experience in social services/discharge planning in a long-term care/Skilled nursing setting
• Working knowledge of Medicare/Medicaid & Insurance
