Bay Path is seeking an experienced, clinically confident Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as our Admissions Nurse, a pivotal role that ensures safe, seamless transitions into our community. This is a per diem, primarily evening-based role designed for experienced nurses who want flexibility without committing to a full scheduled shift or management position. You will be contacted as admissions arise.
While hours are flexible, this position functions in an on-call capacity to support timely admissions.

Position Overview
The Admissions Nurse leads the clinical admission process from hospital or community referral through safe integration into the facility. This role ensures regulatory compliance, accurate clinical documentation, and an exceptional first impression for residents and families.
You are the clinical gatekeeper and transition coordinator.
Key Responsibilities
- Conduct comprehensive admission and readmission assessments
- Review hospital documentation and reconcile orders
- Accurately transcribe and implement physician orders
- Initiate individualized care plans
- Communicate findings with physicians and interdisciplinary team
- Identify clinical risks upon admission (skin, falls, infection, med reconciliation)
- Provide hands-on nursing care as needed during transition
- Ensure timely, compliant documentation to support reimbursement and survey readiness
- Partner with admissions/marketing team to facilitate smooth intake process
- Foster a welcoming and positive customer service experience
Schedule
- Per diem position(Primarily evening admission)
- No routine cart assignment or management shift commitment
Qualifications
- Active Massachusetts LPN or RN license in good standing
- Long-term care, rehab, or post-acute experience preferred
- Strong assessment and documentation skills
- Ability to work independently
- CPR certification (or ability to obtain upon hire)
