- Ambulatory Care Management.
- Assess, reassess, and coordinate care for high-risk patients, ensuring effective person-centered care plans
- Act as a patient advocate, addressing social health needs and collaborating with the interdisciplinary care team
- Engage patients telephonically and assist in navigating services to achieve health goals
- BS or BA in nursing from an accredited college or university
- Minimum of 5 years clinical experience, with 2 years in care coordination / care management within the last 5 years
- Current Minnesota and Wisconsin registered nurse licenses, with one active upon hire and the other obtained within 30 days
