Overview

At Liberty Hospice we understand the unique needs of our patients and families facing terminal illness. That is why Liberty Hospice provides our hospice patients with state-of-the-art care and pain management services, delivered by our specially trained staff with emphasis on strength, dignity and compassion.

We are currently seeking an experienced:

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

(Covering Durham and surrounding areas)

• Implements palliative care program requirements
• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
• Provide clinical findings of that face to face encounter PRIMARY CARE PHYSICIAN, and/or supervising Medical Director/Associate Medical Director
• Communicate with patients/families at all levels
• Communicate with all agency personnel

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program
• Excellent communication and organization skills
