Registered Nurse – Home Health

Sign-on Bonus: $1,500

• Initiate, develop, implement, and revise patient plans of care in collaboration with physicians and other health‑care professionals.
• Supervise care delivered by home‑health aides, licensed practical/vocational nurses, providing instruction and task assignment according to state and federal regulations.
• Perform required supervisory visits and maintain accurate documentation of patient visits per policy and payer requirements.
• Ensure patients meet home‑health eligibility and medical‑necessity guidelines, develop and submit accurate plans of care.
• Communicate timely with agency staff, patients, families, physicians, and payers regarding orders, OASIS data sets, coding requests, and coordination of care.
• Participate in survey/readiness activities, performance improvement plans, and on‑call/weekend rotations as needed.
• Assist with orientation of new personnel, act as a preceptor, and foster teamwork across the clinical team.
• Adhere to utilization management model and infection control standards.

• Current and unrestricted RN license in the state of practice.
• Minimum 1 year of home‑health experience.
• Valid driver’s license, vehicle insurance, and reliable transportation (or public transit).
• Current CPR certification.
• Ability to provide high‑quality care across all home settings regardless of client demographics.

• Independent work style with strong organizational skills.
• Excellent verbal and written communication abilities.

Hourly rate range: $28.27 – $50.48 based on experience and market. In addition to competitive salary, we offer performance‑based recognition, incentive programs, equity stock purchase opportunities, 401(k) matching, and eligibility for other benefits per company policy.
