- Provide and oversee daily clinical care of residents in assigned skilled nursing facilities.
- Conduct comprehensive histories and physicals, diagnose, and develop treatment plans (including diagnostics, prescriptions, and monitoring).
- Offer patient counseling and education on health maintenance and disease prevention.
- Deliver high-quality, patient-centered primary care in the SNF environment.
- Complete accurate, timely documentation of all encounters.
- Be available for phone or secure messaging consultations with nursing staff as needed.
- Use facility EHR systems such as Point Click Care , Matrix Care, and American Health Tech (training provided).
- Collaborate with supervising physicians when applicable, following established clinical protocols.
- Participate in chart reviews and quality audits to support ongoing clinical improvement.
- Maintain reliable transportation and punctual attendance for all scheduled shifts.
- Must be Medicaid-enrolled in the state of employment.
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in good standing.
- Strong clinical skills in primary care, post-acute, or skilled nursing settings.
