AboutThe Role
As a Social Work Assistant, you will support the delivery of high-quality psychosocial services to residents across all levels of care. You will collaborate closely with our licensed social workers, clinical teams, and families to ensure residents receive the emotional, social, and practical support they need to thrive.
Key Responsibilities- Resident support coordination:
Assist with assessments, care planning and follow‑up to promote resident well‑being. - Family communication assistance:
Help facilitate conversations with families regarding care needs, transitions, and available resources. - Documentation and record management assistance:
Maintain accurate, timely notes and support compliance with regulatory standards. - Program and activity support:
Participate in resident engagement initiatives that enhance quality of life. - Transition support:
Assist residents during moves between care levels, offering reassurance and practical guidance.
- Bachelor’s Degree in Social Work or related field:
Experience or education in social work, gerontology, psychology, or related fields. - Strong communication skills:
Ability to build rapport with residents, families, and interdisciplinary teams. - Organizational strengths:
Comfortable managing documentation, scheduling, and follow‑up tasks. - Empathy and professionalism: A genuine commitment to serving older adults with respect and compassion.
