Responsibilities- Review assessment referrals received from various sources in collaboration with the RN and other clinicians, to assess patient needs.
- Assist in coordinating patient care.
- Coordinate service by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, systems complexities, and insurance coverage.
- Maintain patient records, update databases, and ensure accurate documentation of patient information in approved EMR and other platforms.

QualificationsEDUCATION
- Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
LICENSURE/CERTIFICATION
- Preferred Licensed in state: LMSW, LCSW, LPC
- Current CPR; vaccines, if required by a customer hospital. ("Customer hospital" means the hospital to which BHC is contracted to provide mobile assessment services.)
- Must have a valid driver’s license.
EXPERIENCE
- Preferred 1-3 years work-related experience
