Community Nursing Sister / Qualified District Nurse

Dunstable and Leighton Buzzard, within the Integrated Primary Care Hub.

As part of the primary home care team, you will focus on providing comprehensive palliative care to patients s role offers an opportunity to develop expertise in end‑of‑life care, support self‑care for long‑term conditions, promote health, and contribute to innovative projects that enhance patient service delivery and staff wellbeing.

• Deliver and support all aspects of end‑of‑life care at home.
• Manage a caseload of palliative patients, ensuring continuity of clinical care.
• Facilitate self‑care programmes and health promotion activities for patients with long‑term conditions.
• Collaborate with the team to develop and implement innovative projects that improve service delivery and staff wellbeing.
• Maintain accurate and timely patient records in accordance with NHS standards.

• Experience in adult palliative care and community nursing is desirable.
• Excellent communication, time‑management and clinical knowledge are essential.
• A full driving licence and access to a vehicle are required to carry out the duties.
• Willingness to work a shift pattern covering 7 days a week, full time.

This is a full‑time, 7‑day‑a‑week role. We encourage flexible working arrangements to suit both staff and patient needs. Discuss possible job share, part‑time or other flexible patterns with our team.

The Trust offers a relocation scheme of up to £10,000 for removal and associated expenses (subject to change and terms and conditions apply).
