We are currently seeking an experienced:

HOSPICE PSYCHOSOCIAL SUPPORT SPECIALIST II

Responsibilities
  • Provide assistance in psychosocial program development, training and processes.
  • Assist with overall administrative functions as needed.
  • Review, audit and aggregate findings for appropriate documentation and processes of psychosocial disciplines in patient care and best practice.
  • Keep abreast of Medicare requirements, working knowledge of Conditions of Participation and changes required for psychosocial disciplines and educate on the same.
  • Assist in review of policies and procedures for Liberty Hospice.
  • Monitor paper forms and other documentation process with team to maintain continuity.
  • Monitor psychosocial monthly reports as assigned and provide information to Director of Social Services and individual Administrators/Clinical Managers.
  • Report to the Director of Social Services and assist the Psychosocial Support Coordinator in other administrative processes.
  • Gather statistical information to complete licensure reports and end of the year reporting.
  • Identify ways to streamline operational procedures; assist in the orientation and training of new psychosocial employees as assigned.
  • Assist in team building for individual employees and staff as assigned.
  • Promote compliance with regulations and standards particular to state, federal, local and accrediting bodies AHHC, COPS as well as Liberty Hospice standards, policies and procedures.
  • Cover support process for discipline-appropriate staff when on PTO or increased needs are evident.
  • Must be able to travel to distant offices to provide ongoing coverage and support as assigned.
  • Coordinate setup for assigned psychosocial trainings including set up, location, attendee communication, refreshments and supplies, and coordination with speakers/educators.
  • Assist in QAPI process for improvement projects for psychosocial–increased best practice patient care.
  • Implement programs to develop relationships with organizations to build community coalitions and promote health awareness and managed care.
  • Assist with the creation of social media posts.
  • Assist and develop marketing tools and educational brochures for patient and family needs for psychosocial concerns.
  • Educate and encourage bereavement coordinators and volunteer coordinator in partnering with community-based organizations.
  • Support the psychosocial team to understand and engage in their respective communities.
  • Provide hands-on support in continued development of community-based, Liberty-supported grief program.
  • As appropriate due to need, professional qualifications and determined by Director of Social Services: assist with duties of disciplines in need of assistance due to PTO or increased case load.
  • Assume duties as described in individual job descriptions for providing support coverage of assigned disciplines.
  • Connect with teams and psychosocial disciplines to ensure continued engagement through positive experiences.
  • Set example of proper behavior and accountability to employees and serve as a role model.
  • May carry case load appropriate to discipline knowledge, within scope of practice and need of office assigned (this would be a portioned case load).
  • Maintain policy required education by attending in and out of office workshops and completion of yearly education.
  • Attend Liberty Hospice Psychosocial employee meetings.
  • Support needs and communication coordination with the Director of Social Services and Psychosocial Support Coordinator.
  • Exhibit professional verbal and written communication.
  • Communicate with all agency personnel, staff, volunteers, other Liberty employees, patients’ family members / caregivers, and all outside customers, in a professional and positive manner.
