Schedule

Day shift, 5x8 hours, typically Monday to Friday, with varying hours based on the needs of the Home Care office.

Job Description

The Skilled Home Care Nurse will provide essential home health care to patients, focusing on a variety of medical diagnoses including cardiopulmonary assessment, wound care, and post-operative care. Nurses are expected to conduct 6-8 home visits per day, performing necessary nursing procedures and documentation in accordance with established protocols.

Responsibilities

• Conduct patient assessments and perform necessary medical procedures such as wound care and IV management.
• Administer medications and treatments as prescribed by healthcare professionals.
• Utilize medical equipment and technology for patient care, including glucometers and pro-time machines.
• Maintain accurate patient records and documentation through electronic charting systems.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

Required Experience / Certifications / Licensure

• At least 1 year of skilled home care experience required; private duty nursing experience alone is not acceptable.
• OASIS training and experience is mandatory.
• Current Florida nursing license or compact license is required at the time of application.
• Certifications:
  
  BCLS required.
• COVID-19 vaccination or exemption required; must also have flu shot or declination.
• Valid driver's license and access to transportation for home visits.
