Job Description
American Traveler is seeking a Physical Therapist for a Home Health role requiring 2 years of experience and BLS certification.
Responsibilities- Home health setting serving a variety of patient populations including cardiopulmonary, geriatrics, neuro, ortho, and oncology
- Home skilled visits with a minimum of 6 patients per day, which may increase at manager discretion
- Day shift, 5x8 hours, 8:00 AM to 5:00 PM
- Every third weekend coverage is required
- Caseload involves assessment, documentation, equipment management, and patient/family education
- Uses Bay Care Forms, Bay Care Scheduling, and Ace Timesheet for documentation
- Self-scheduling of visits and hours based on home care office needs
- Locals accepted at the same rate
- Responsibilities include performing assessments, developing home exercise programs, managing equipment and devices, and educating patients and families
- Knowledge required in safety, injury prevention, and recognition of abuse indicators
- Self-scheduling allows flexibility according to patient and office need
- Dress code is solid color scrubs of any color
- Electronic charting and documentation are required for all home visits
- Active state PT license required if applicable
- BLS certification required
- Minimum 2 years of recent home health experience
- OASIS training preferred
- Valid driver's license and personal vehicle required for home visits
- Ability to use or learn portable equipment including laptops, MiFi devices, and portable printers
- COVID vaccination or approved exemption required
- Flu vaccination or declination with mandatory masking required
