Lance Soft is seeking a LPN / LVN Home Health for a travel job in Dunedin, Florida.

Setting: Skilled Home Health (Medicare, Medicare Advantage & Commercial)
Schedule:

• Home visits: 6–8 patients per day
• Shifts vary based on office needs:
  • Mon–Fri 8:00a–5:00p or 9:00a–6:00p
  • Thurs–Mon or Fri–Tues rotations
  • Alternating weekends
• Visits and hours are self-scheduled

Required Experience & Skills:

• Skilled home health experience required (private duty only not accepted)
• OASIS-trained with hands-on experience
• Strong IV and wound care skills
• Experience with general medical and high-risk diagnoses (HF, COPD, MI, Pneumonia, Stroke)
• Post-operative care experience

Clinical Responsibilities:

• Cardiopulmonary assessments
• Basic and advanced wound care (ABI compression, wound vacs)
• Pleur
  
  X/Asept drain care
• Foley catheter care and changes
• Staple/suture removal
• Glucometer and pro-time machine use
• SQ/IM injections, venipuncture lab draws
• IV therapy, PICC/Midline care, SQ port access/de-access

Requirements:

• BLS/BCLS
• Minimum 1 year experience
• Valid driver’s license and personal vehicle (required for home visits)
• COVID vaccination or approved exemption
• Flu shot or declination (mask required if declined)

Tools & Documentation:

• Laptop, MiFi, portable printer provided
• Documentation systems: Bay Care Forms, Bay Care Scheduling, Ace Timesheet

Dress Code:

• Any solid-color scrubs
