We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs.
Responsibilities- Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.
- Order diagnostics, prescribe medications, and educate patients, families, and staff.
- Collaborate with care teams to ensure seamless, patient-centered care.
- Maintain accurate documentation and support quality initiatives.
Current MN NP license, board certification, and preferred 1+ year experience in geriatric or post-acute care.
