We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care, and Assisted Living Facilities.
- Perform patient assessments, manage acute and chronic conditions, and develop treatment plans.
- Order diagnostics, prescribe medications, and educate patients, families, and staff.
- Collaborate with care teams to ensure seamless, patient-centered care.
- Maintain accurate documentation and support quality improvement initiatives.
Current MN NP license, Board Certification, 1+ year related experience preferred, strong geriatric and primary care background, excellent communication skills.
