We’re looking for a Physician Assistant (PA-C) who believes in person‑centered, place‑based care. Instead of a rushed clinic setting, you’ll have time to connect with patients in their own environment where your care can truly change outcomes.

• Provide routine and follow-up primary care to older adults
• Collaborate with care teams and support staff on treatment planning
• Monitor chronic conditions and acute medical concerns
• Use EMR to document care clearly and consistently

• MN PA license in good standing
• Experience in geriatrics, care transitions, or chronic disease management
• Compassionate and collaborative
• Valid driver's license and auto insurance
