Social Worker, Mental Health

Overview

Provides social work services and discharge planning to patients and families in the healthcare setting to meet individual health and psychosocial needs in the promotion of wellness and cost-effective outcomes. This position also transitions patients to alternate levels of care and provides crisis intervention as needed.

• A bachelor degree in social work from an accredited social work program

• Current State Social Work Licensure

• Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
• Proof of current motor vehicle insurance.
