Case Manager II - Targeted Case Management (St. Louis County)
People Incorporated is seeking Case Managers to join our Targeted Case Management team in St. Louis County to work with clients diagnosed with chronic mental illness. We support people in connecting with resources to live independently and help them develop self‑awareness to make positive plans for the future through frequent community meetings. Case Managers develop long‑term relationships with clients and operate autonomously, creating their own schedules.
We prioritize work‑life balance and individualized, high‑quality care. Case Managers carry average caseloads of approximately 30 clients, allowing meaningful engagement and effective service coordination.
Pay RangeBase pay: $24.25/hr – $25.75/hr.
Hours & LocationFull‑time, Monday – Friday, regular business hours. Remote with required physical presence in St. Louis County.
Hiring Bonus$1,000 to external candidates.
MainJob Duties
- Facilitate in‑person appointments with clients in Duluth, Minnesota.
- Perform functional assessments that lead to individual plan development and result in the client receiving necessary mental health and related services.
- Conduct regular reviews of plan effectiveness and make adjustments to maximize client assistance and support while monitoring service effectiveness.
- Assess client needs and ensure continuity of care.
- Make ongoing referrals to resources based on assessment and current client need.
- Maintain necessary daily records for continuity of service and reports to funding agencies.
- Document all interventions, interactions, and relevant information to the treatment plan consistently with professional and organizational standards.
- Attend client monthly meetings to monitor services.
