Job Description
Division/Department: Human Services Division
- Behavioral Health
Location: Min No Aya Win Human Services Center & Center for American Indian Resources
Reports To: Behavioral Health Coordinator/Clinical Director
Probationary Period: 90 days – Full Time; 120 days – Part Time
FLSA Status: Non-Exempt (Hourly)
Classification: Child-Related CA
Rate of Pay: $20.45-$23.01
Responsibilities
- Monitor and process referrals for all Fond du Lac Behavioral Health Services.
- Schedule intakes and ensure completion of required intake paperwork.
- Develop and refine processes to facilitate timely referral processing and intake scheduling.
- Maintain accurate and timely documentation in an electronic health records system.
- Prepare and submit grant reports and required monthly/quarterly reports.
- Stay informed about area resources and guide clients and families to appropriate services within Fond du Lac Human Services and the surrounding region.
- Collaborate with internal and external multidisciplinary teams to enhance services for clients and families.
- Participate in community outreach and educational activities to promote awareness and prevention of mental health challenges.
- Provide crisis intervention, de-escalation support, and assist in implementing crisis and safety plans while supporting the mobile crisis team.
- Conduct and contribute to in-service training sessions for Human Services staff on service delivery and best practices.
- Engage in clinical consultation, staff meetings, and seek supervisory support as needed.
- Maintain professional development through continuing education to uphold program certification or licensure requirements.
- Deliver mental health case management services to children/adolescents with severe emotional disturbances (SED) and their families.
- Provide case management for adults diagnosed with serious and persistent mental illness (SPMI).
- Must work a flexible schedule to contact candidates after hours.
- Evening and occasional weekend work.
- Appropriate/Professional work attire is required.
- Must attend all mandatory department trainings.
- Ensures confidentiality of financial records and all records for employees and/or clients.
- Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
- Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
- Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
- Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
- Maintains a clean and organized work area.
- Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.
Qualifications
- Valid U.S. Class D Driver’s license with Fond du Lac Insurability is required.
- Bachelor's degree in social work, psychology, counseling, or a related field is preferred. Equivalent experience in behavioral health or human services may be considered.
- Previous experience in behavioral health intake, case management, or related services is preferred.
- Knowledge of computer applications to include word processing, data bases and/or data entry, Spreadsheets, and Microsoft Outlook is required.
- Knowledge of Native American communities, culture, customs, and beliefs is preferred.
- Experience working with Native Americans clients, communities and is able to demonstrate cultural sensitivity is preferred.
- Ability to communicate effectively orally and in writing is required.
- Ability to work independently and establish priorities is required.
- Attention to detail and accuracy is required.
- Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
