Home Health Licensed Practical Nurse (LPN/LVN)
Aveanna Healthcare is hiring a Home Health Licensed Practical Nurse (LPN/LVN). The role involves providing and documenting skilled nursing care in accordance with the care plan and physicians’ orders for each patient, while maintaining confidentiality and professional boundaries.
Responsibilities- Deliver quality patient care in compliance with physician orders under the direction of a Registered Nurse.
- Perform the continuum of hands-on client care through documentation and timely communication with the client s physician and other caregivers.
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk.
- Educate clients and their family members based on the client s specific needs.
- Provide home care using skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
- Active LPN license in the state of application.
- Current CPR certification (with hands-on component).
- TB skin test current within the last 12 months.
- Reliable transportation.
- Ability to read and write English.
- Medicare Skilled Nursing experience.
- Basic understanding of Oasis.
- 1 year LPN experience in a home health or health care setting.
- Home Care Home Base experience is a plus.
Physical Requirements
- Must be able to function in diverse environments; may involve exposure to allergens and other conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases.
- Must be able to travel and lift up to 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting; able to climb stairs and reposition patients safely.
- Must have visual and hearing acuity, and a strong sense of smell and touch.
