Program Manager – Community Assistance Program

We’re seeking a self-motivated, licensed clinical professional to lead and support a multidisciplinary community-based team that serves individuals with co-occurring (mental health and substance use disorders). This role requires both strategic leadership and direct field involvement to promote integrated treatment and real-time assessment of client progress and providing one on one counseling.
Some fieldwork required.
- Provide hands-on clinical supervision and field support with team members.
- Engage directly with clients for support (to include transportation as needed/clinically appropriate).
- Facilitate group sessions in community settings to promote engagement and stability.
- Lead integrated treatment planning and care coordination in collaboration with Qualified Mental Health Professionals (ITCD Specialist), peer specialists, case managers, providers and nursing staff.
- Ensure treatment fidelity and quality based on Integrated Treatment for Co-Occurring Disorders (ITCD) principles as identified from Department of Mental Health.
- Conduct regular documentation reviews and progress audits to guide clinical decision-making.
- Champion trauma-informed and person-centered care approaches across the team.
- Cultivate partnerships with local community resources and supports.
- Master’s degree in Counseling, Social Work, Psychology, or related field.
- Active and unrestricted clinical license (e.g., LPC, LCSW, LMFT).
- Demonstrated expertise with ITCD model and evidence-based practices for dual diagnosis such as MI (Motivational Interviewing).
- Valid driver’s license and willingness to use personal vehicle for client transport.
- Experience with group facilitation and community outreach required.
- Comfortable with electronic health record systems and case documentation.
- Strong leadership, organizational, and communication skills.
- Commitment to equity, inclusion, and culturally responsive care.
- Background in harm reduction, recovery-oriented services, and housing-first models.
- Ability to lead with humility, clinical insight, and creative problem solving.
- Strong diagnostic judgement across mental health and substance use domains.
- Ability to quickly adapt care strategies in dynamic community settings.
- Comfortable making real-time decisions while balancing safety, ethics, and client autonomy.
- Skilled in modeling best practices during street outreach or in-home visits.
- Capable of mentoring team members with supervision and encouragement.
- Confidence navigating unpredictable environments while remaining calm and client-focused.
- Fluent in trauma-informed dialogue and motivational interviewing techniques.
- Sensitive to cultural, socioeconomic, and racial identities that shape the client experience.
- Skilled in de-escalation and rapport-building with diverse populations.
- Able to see the “whole picture” in team dynamics, treatment planning, and outcome tracking.
- Leads with humility, curiosity, and empathy.
- Invested in reflective practice and continual feedback loops.
- Inspires confidence through clear expectations, appreciation, and clinical wisdom.
