Responsibilities- Be part of a dynamic team to provide strength-based community case management services to individuals who possess a mental health diagnosis.
- Development of recovery-oriented treatment plans.
- Collaboration with assigned treatment team.
- Link with community-based resources and services.
- Advocacy and support for individual recovery journeys.
- Coordinate with crisis services through an on-call rotation.

- Bachelor’s Degree in Psychology, Sociology, Human Services, Criminal Justice, or related field.
