Overview
Behavioral Health Treatment Coordinator (BHTC) provides an array of services delivered by community based, mobile individuals or multidisciplinary teams of professionals and trained others. Services address the individualized mental health needs of the client.
Qualifications- Degree in a behavioral sciences field preferred but not required – LSW or LPC preferred
- 2-5 years’ experience in mental health or developmental disability environment preferred
- Holds or must obtain QMHS certification
- Ability to work flexible hours
- Proven evaluation and organizational skills
- Strong written and oral communication and public speaking skills
- Proficiency in computer skills, such as working with EMR systems, productivity software, and customized databases
- Good interpersonal skills with ability to relate well with persons from multi-cultural and diverse socioeconomic backgrounds
- Meet and maintain all State of Ohio background investigation requirements, and FBI background investigation if applicable
- This is a Direct Service position – must have reliable transportation, valid Ohio driver’s License with a clean driving record, and proof of insurance
- Maintain professional client relationships, including the ability to be consistent with clients and treat everyone equally with respect and dignity.
- Assist patients in implementation of a care plan tailored to the patient’s needs and issues, designed to improve basic functioning in daily and community living.
- Identify and refer to resources specific to the needs and issues of patients.
- Serve as a liaison, patient advocate and coordinator among those involved in the patient’s care including SHC and other community referral sources.
- Provide ongoing education to patient and family members in the form of verbal and/or written consultation or educational materials.
- Carry a case load and provide timely and accurate record keeping and reports as required for assisting patients.
- Assist patients in attendance and compliance with medical and mental health appointments.
- Coordinate scheduling of assessments of patients, medical or other.
- Assist patients in accessing natural support systems in the community and identify linkages to formal community services.
- Collaborate with therapists, nurses, nurse practitioners, physician’s assistants, and psychiatrists regarding patient care.
- Responsible for setting and enforcing appropriate boundaries with clients and co-workers.
- Represent Secure Haven Behavioral Health in a professional manner in the community, with clients and in all areas.
- Evaluate potential abuse, neglect, and risk of danger to self and others, and assist patients and families in securing the least restrictive environment for ensuring safety.
- Manage psychiatric emergencies by determining the level of risk and initiating and coordinating effective emergency care.
- Facilitate groups aimed at improving basic functioning such as life skills or communication.
- Create a climate of patient-centered care to include confidentiality, empathy, respect, and collaboration.
- Deliver patient care with respect to culture, ethnicity, gender, religious beliefs, age, and physical health problems.
- Ensure quality of care through consultation, collaboration, continuing education, certification, and evaluation.
- Attend staff meetings, trainings, and conferences as assigned.
