Job Description Summary

*** This is a part-time (16 hours/week) position working in Med Safety at Dublin Methodist Hospital. Previous hospital experience is strongly preferred and Med Safety experience is encouraged. Wednesdays are required with the second day being flexible.***

Medication Safety Position Description
  • Leads medication safety strategy for accountable site(s) in collaboration with system medication safety team.
  • Facilitates and manages activities of the hospital’s Medication Safety Committee.
  • Utilizing medication event-reporting data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient safety.
  • Responsible for ensuring compliance with national patient safety recommendations and regulatory standards related to safe medication use.
  • When a significant medication error occurs, participates in the root cause analysis evaluation and recommends system changes as necessary. Leads and participates in failure mode and effects analysis on medication use systems and performance improvement projects as necessary.
  • Reviews high volume, high risk, and problem prone or critical aspects of care processes for prioritization of potential medication safety improvements or initiatives.
  • Ensures that healthcare providers are educated in safe medication-use practices. Collaborates with different departments and committees focusing on patient safety and serves a preceptor to Pharm.

    D. candidates and pharmacy residents.
  • Integrates medication safety improvement initiatives, activities and changes with clinical and operational department programs and committees (i.e., P & T Committee, Patient Safety Council, and Pharmacy/Nursing Committee).
Minimum Qualifications And Specialized Knowledge
  • Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required)
  • Current Ohio pharmacist license (Required)
  • Basic Life Support Certification—current or obtained during the orientation period (unless >50% of scheduled work hours, as determined by the manager, are completed off‑site or not in a patient care setting)
  • Hospital experience (strongly preferred)
  • Previous Med Safety experience
  • ASHP accredited residency or equivalent pharmacy practice experience
  • Pharmacy Board Certifications or other certifications
  • Advanced-level degree in healthcare field
  • Collaborative practice experience
  • Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)
  • Pharmacokinetic drug dosing experience
  • Drug therapy management
  • Doctor of Pharmacy (Required) RPH – Registered Pharmacist – Board of Pharmacy Specialties

Acute Care/Infusion
  • 50% – Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy‑related devices in accordance with standards of practice and Ohio Health pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication.

    Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
