Clinical Consultant Social Worker (MSW/LSW) Casual

This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at the assigned hospital. Responsibilities include providing services to patients, families, and significant others through biopsychosocial assessments, psychiatric diagnoses, treatment plans, therapy sessions, and discharge planning. The role receives referrals from interdisciplinary team members and must demonstrate knowledge and skills necessary to provide appropriate care across life cycles.

• 30% – Clinical Assessment:
  Conduct patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret biopsychosocial functioning information; develop care plans, including discharge plans; identify abuse, neglect, or exploitation cases; provide crisis intervention and linkage to community resources.
• 30% – Intervention Management:
  Develop intervention plans based on assessment; employ clinical intervention skills through individual, family, or group modalities, including treatment planning, discharge planning, supportive counseling, crisis intervention, and educational counseling.
• 25% – Population Management:
  Coordinate clinical and financial outcomes for an assigned population; collaborate with interdisciplinary staff and external organizations; monitor quality and type of services; liaise with payers to manage post‑discharge resources.
• 10% – Documentation and Regulatory Compliance:
  Thoroughly document assessments and interventions in patient medical records and information systems per department standards and regulatory requirements.
• 5% – Community Services Awareness:
  Maintain awareness of hospital and community services; provide information and facilitate linkages between services and patients/clients; perform additional appropriate functions.

• Master's Degree (Required)
• DL – Driver’s License
• LSW – Licensed Social Worker (Social Work Certification and Licensure Board)

State Driver’s License. Interview, assessment, organizational, and problem‑solving skills. Ability to identify appropriate community resources on assigned caseload and collaborate with patients, families, multidisciplinary team, and community agencies to achieve desired patient outcomes. Demonstrates patience and tact when dealing with patients, families, and staff. Performs duties in a manner that promotes quality patient care, customer service/satisfaction, safety, cost efficiency, and continuous quality improvement.
