Our stunning community at Emerald Valley is seeking a Wellness Program Coordinator to join our team of senior living heroes!

Tue, Wed, Thur, Fri, Sat 9:00am-5:30pm

At MBK, we believe that every resident has the potential to experience joy and pleasure, connection with others, purpose, and self‑esteem at every point on the spectrum of the cognitive changes we call dementia. The purpose of the Wellness Program Leader in memory care is to uncover and develop each resident’s potential to have these positive daily experiences and to grow.

This goal is achieved by creatively planning and implementing a daily Wellness Program using the structures and content provided by MBK.

• Understand each resident’s wellness needs and document them in the Service Plan –20%
• Review the Lifestyle Survey for each new resident prior to move‑in and create a plan to welcome and integrate the resident into the Wellness Program beginning on move‑inday
• Identify resident’s interests, needs, and potential in the first 30 days after move in and incorporate them into the neighborhood’s Wellness Program plan
• Document Wellness Program services provided in each resident’s Service Plan
• Ensure that every resident has large group, small group, and/or individual experiences included
