SummaryThe Licensed Practical Nurse (LPN) will provide health services and direct care to individuals with disabilities living in a residential program. In addition, the incumbent will provide advocacy and liaison with community health care providers, monitoring medical records, and medication administration.
ScheduleSunday 9:00 AM - 5:00 PM, Monday 3:00pm-11:00pm, Saturday 9:00 AM - 4:00 PM (23 hours)
Essential Duties And Responsibilities Include- Provide direct specialized nursing and other day-to-day care to individuals.
- Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements.
- Function under the supervision of the residential manager, in collaboration with the registered nurse and Director of Health Care.
- Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care.
- Provide in-service education to individuals and/or staff on health issues.
- Maintain knowledge of the Department of Public Health Medication Administration Program (MAP) current policies and procedures.
- Administer medications to assigned individuals following MAP policies and procedures
- Provide quality assurance reviews of medication systems and MAP compliance.
- Provide support and assist individuals with all activities of daily living.
- Provide care to individuals such as checking vital signs, checking their weight, applying creams/ointments, etc.
- Maintain the comfort, privacy, and dignity of each individual in the delivery of services to them.
- Observe residents for changes in condition or behavior and promptly report these changes to the Residential Manager.
- Graduation from an accredited and approved licensed practical nurse program.
- Must have current Massachusetts Nursing license.
- Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.
- Excellent written and oral communication skills.
- Excellent organizational and problem-solving skills.
- Ability to foster positive relationships.
- Must have basic to intermediate computer-level competency.

- While performing the duties of this job, the employee is frequently required to climb stairs.
- The employee must be able to drive a vehicle.
- Must have a normal range of eyesight with or without corrective lenses.
- Must have a normal range of hearing with or without corrective equipment.
- Must be able to lift or pull up to 40 pounds.
