Een betrokken zorgorganisatie in Friesland zoekt een Zorgcoördinator voor 24 tot 32 uur per week. In deze rol ben je de schakel tussen de manager en het team, ondersteun je het management en coördineer je zorgprocessen. Je hebt een afgeronde HBO-opleiding in de zorg en ervaring met cliënten met complexe problematiek. Bij ons krijg je de ruimte voor eigen inbreng en ontwikkeling in een klein team met een informele werksfeer.
