- Minimum: HS Diploma + 12 college credits in a related field
- Preferred: Bachelor's degree in psychology, social work, counseling, or related field
- 1+ year of direct client care experience (internships/volunteer work considered)
- Experience with mobile services and case management preferred
- Strong communication and computer skills
- Valid Driver's License
- Ability to meet physical requirements of the role, with or without accommodation
- Provide community-based Blended Case Management services to children and adults in Central Bucks County
- Meet individuals and family members in their homes and communities to assess needs and connect them to vital services, resources and programs to positively impact an individual's overall well-being
- Collaborate with LVF's multidisciplinary team and external providers
- Maintain accurate, recovery-oriented documentation in our Electronic Health Record (EHR)
