Description

The social worker is a member of the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs, and help provide both counseling and casework needed to meet these goals.

• Participates in the preparation, evaluation, and implementation of the interdisciplinary plan of care.
• Assures that the highest quality of social services is offered to patients/families and that goals of continuity and comprehensiveness of care are achieved.
• Provides psychosocial and emotional support to the patients/families.
• Maintains appropriate documentation in the hospice patient chart.
• Attends Interdisciplinary Team meetings for case review and development of plan of care.
• Participates in in-service education programs.
• Facilitates appropriate community resources and provides casework.
• Act as a resource for other interdisciplinary team members at the direction of the team manager.
• Completes assessments timely per policy.
• Accepts and performs other related duties and responsibilities as required.

• The incumbent in this position will have no direct reports.

• At least 18 years of age.
• Master of Social Work Degree (MSW) from an accredited school.
• Clinical experience relevant to psychosocial needs of patients and families.
• One (1) year of social work experience in a health care setting.
• Current driver's license and proof of insurance required.
• Basic Life Support (BLS) required. Travel required.

• Knowledge of stress of repeated loss and ability to provide support to the rest of the team.
• Knowledge of community resources and ability to utilize these resources.
• Demonstrates the ability to use good listening and communication skills.
• Able to multi-task effectively.
• Capacity to relate and work as a member of an interdisciplinary team.
• Must be a licensed driver with an automobile that is insured and in good working order.
