Overview
Physical Therapist Assistant - Home Health - PRN
Meraki Rehab Partners is seeking a PRN Physical Therapist Assistant (PTA) to join our Home Health team. We provide PT, OT, ST, and RT services in post-acute settings. Responsibilities- Provide therapy services established by the Physical Therapist (PT) in the patient Plan of Care.
- Provide therapy services under the supervision of a PT as determined by the Registered Therapist and in compliance with State and Federal guidelines.
- Consult with PT regarding changes in treatment or patient condition in a timely manner.
- Complete documentation including daily notes and assist the PT in creating progress notes, discharge summaries, or monthly reports as needed.
- Submit billing, payroll, and expense documentation in a timely manner.
- Communicate with patients, families, referring physicians, and other team members.
- Attend continuing education seminars/workshops to stay current with clinical skills, programs, or techniques.
- Participate in care planning meetings, rehab meetings, and family training to communicate patient progress as requested by PT, Director of Rehab, or Rehab Resource Consultant.
- Follow organizational and facility policies and procedures to ensure safety for employees and patients.
- Ensure equipment is in good working order prior to delivering care; report damaged equipment as needed.
- Maintain a clean environment and participate in required meetings and in-services as designated.
- Coordinate treatment goals with other team members.
- Maintain valid licensure, certification, or equivalent as required by state agency.
- Minimum of an Associate of Arts in Physical Therapy Assistant (or equivalent).
- Licensure or Certification as required by state guidelines; CPR Certification.
- Experience:
1 year Physical Therapy Assistant experience preferred; 1–2 years in a residential/assisted living or skilled nursing facility preferred. Graduates in PTA program will be considered.
