The Nephrology RN Care Manager works with an integrated specialty care team to support patients with CKD/ESRD, focusing on outcomes, medication adherence, home dialysis, transplant education, and high‑risk care management.
- Enroll and manage a caseload of high‑risk CKD/ESRD patients.
- Conduct comprehensive clinical assessments via phone, including medical, behavioral, pharmaceutical, and social needs per policies and procedures.
- Maintain proactive communication with nephrologists, PCP offices, and clinical partners for timely escalation and service coordination.
- Inventory and reconcile medications with pharmacists and prescribers; encourage adherence through frequent contact.
- Assess home safety and social determinants of health, escalating resources where appropriate.
- Facilitate care across settings (home, hospital, skilled nursing facilities) and during transitions to minimize readmissions.
- Provide education on CKD, ESRD, dialysis, and comorbidities; assess patient understanding and support self‑management.
- Serve as the initial point of contact for escalations and provide clinical oversight to care coordinators, delegating tasks as appropriate.
- Coordinate with dialysis providers to ensure seamless transitions.
- Educate patients on proactive care decisions, including advance care plans and ESRD treatment modalities.
- Analyze data from predictive modeling tools to identify patients for care management.
- Assist with evaluating and amending case management policies and procedures.
- Review population health dashboards to address care gaps (e.g., wellness visits, labs).
- Registered Nurse license in Illinois.
- 2+ years of experience in case management or caring for CKD/ESRD patients.
- Telecommunication experience with patients.
- Basic Life Support (BLS) certification (required).
- Preferred:
Certified Case Manager (CCM) certification. - Computer proficiency:
Microsoft Office, typing 40 wpm. - Strong analytical, critical thinking, and problem‑solving skills.
- Effective verbal communication and empathy.
- Team orientation and ability to adapt quickly.
- Experience managing high‑risk patients and coordinating care plans.
- Preferred management experience.
- 3+ combined years of education, experience, or certification.
Base pay range: $79,040–$104,000, determined by skills, experience, education, certifications, location, and equity. Additional compensation may include shift differentials, bonuses, and incentives.
