Overview
Community Partners is a non-profit organization that provides critical supports and services for individuals and families living with developmental disabilities and/or behavioral health challenges. An in home personal care provider works with children and adolescents with intellectual disabilities. The provider will work 1:1 with the individual on goals established as part of the individuals In Home Supports plan. Goals may focus on but are not limited to increasing independence with activities of daily living, increasing social/communication skills and gaining life skills.
ResponsibilitiesThe personal care provider supports individuals in their home setting in partnership with families and care teams. Responsibilities may include implementing individualized goals, providing direct support, and helping the child or adolescent develop daily living and life skills while focusing on safety and well-being. Transportation will be a requirement of this position and will be used to facilitate community connections and activities aligned with the individual’s plan.
Qualificationsand Attributes
• Experience working with individuals with disabilities, in particular autism, is preferred.
• Energetic, observant, and a strong communicator who can establish and maintain community connections on behalf of the child they support.
• Comfortable identifying support needs and collaborating with families and professionals to support goals.
We currently have a number of individuals in need of personal care support across Strafford County, varying in age, levels of support needed and number of hours needed per week. Transportation is a requirement; candidate must possess a valid driver’s license, a clean driving record and proof of insurance. All candidates must pass a background check which includes, but is not limited to, criminal and driving records to be considered for the job.
