- Must live greater than 50 miles from Dover, NH.
Job Requirements
- 5x8 shifts, 40 hours/week.

- Position in Blood Bank unit
Unit Specific Information
- Primary Source Verification of professional license and/or certification required within 30 days of start date and updated upon expiration.
- N95 fit test required within 365 days to start date and updated annually; no fit testing onsite except at specified locations.
- Documentation of MMR vaccination or proof of immunity required.
- Strongly preferred Hepatitis B vaccination or proof of immunity.
- Influenza vaccine required during flu season (Sept 15 - April 30) with no exemptions.
- Documentation of TB skin test or blood test within 3 months of start date and updated if >6 month break.
- Physician statement within 365 days of start date and updated if >6 month break.
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
- MGB Resume including past 7 years of work experience required for submission.
- Respirator Medical Clearance Attestation required if fit test is required.
- 2 recent references required, minimum 1 supervisor or above in skillset.
- MGB Skills Checklist required for submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- MGB Education Verification required.
- MGB SSN Trace required.
- MGB National Sex Offender Check required within 30 days of start and updated annually.
- MGB Exclusion Checks required within 30 days of start and updated annually.
- MGB New Hampshire State Checks required within 30 days prior to start date and updated if >30 day break.
- TB Screening Form to be completed prior to start and updated annually.
